Select the Account Maintenance menu item. |
SUITS displays the Employer Maintenance screen. |

Select the Extension History icon. |
SUITS displays the Extension History lookup screen. |

Enter Search Criteria as warranted and click the ‘Search’ button. |
SUITS displays results below in the Search Results panel. |

View the Search Results and click the ‘Export to Excel’ button if desired. |
