Purpose: The Employer Registration Wizard is the screen housing the wizard where employers are registered in SUITS. This wizard contains the following steps:
1. Initial Questions
2. Business Information
3. Business Address
4. NAICS Classification
5. Ownership Information
6. Review and Submit
The following information is needed in order to register an employer in SUITS:
•Contact information for the employer.
•The date wages were/will be first paid for services performed in State.
•Type of employment.
•The Federal Employer Identification Number (FEIN).
•The business entity type (for example: corporation, partnership, etc.).
•The legal and trade or doing business as (DBA) names for the business.
•The state in which the business was formed.
•The date of formation or incorporation of the business.
•Business owner or officer information, including SSN and owner home address.
•501(c)3 and/or corporate charter, if applicable (in PDF or Microsoft Word .docx format for upload).
This wizard is viewable in by internal users, agents, and employers, depending on the user roles.
In the internal Staff Portal, staff members can register an employer as staff. This might occur if an employer does a paper registration; then the staff member who may receive the paper registration can input the information into SUITS.
In the Agent Self-Service Portal, agents can register staff members as agents. This might occur if an agent has a relationship with an employer who is not already in the SUITS system, but wants to be added.
In the Employer Self-Service Portal, employer can register themselves as well. At the end of their registration, they would be taken directly into the Employer Self-Service Portal. This is the preferable route for employers to take.
Employer registration can also be the entry point for an experience transfer; those steps are added to the end of the Employer Registration Wizard when applicable.
