What happens next: |
• The Employer has submitted a legal name change request to the UI agency using the Self-Service portal. • SUITS sends the UI agency a work item to “Review the Legal Name Change”. |

From the Home screen, click the Account Maintenance left navigation menu item. |

Click the Change Business Name link. |
SUITS displays the Change Name Maintenance screen.

Click Change DBA Name to change the “Doing business as” name. All steps remain true except there is no workflow generated for changing the DBA name as opposed to a Legal Name |

Click the Change Legal Name link. |
SUITS displays the Change Legal Name Wizard. |
This is Step 1 of the Wizard – Change Legal Name. |

Enter a New Legal Name in the Change Legal Name panel. |

Select the Yes or No radio button for the question: “Is the Legal Name change related to a merger/acquisition or business reorganization?” |

Click the Select Files button to upload the Articles of Amendment or any other supporting document for the name change. |
SUITS displays your computer’s documents folder. |

Select the file. |
SUITS uploads the document to your information request. |
You will know the file is uploaded to the request because the file name will be displayed under the Select Files button. |

Click the Next button. |
SUITS displays the Summary screen. |

To update or edit the request, click the Previous button and follow the Wizard’s prompts. |

Click the Next button if you are satisfied with the request. |
